Neuroleptic Malignant Syndrome (NMS)

rare neurological disorder most often caused by an adverse reaction to neuroleptic or antipsychotic drugs.

Manifestations:

  1. muscle rigidity
  2. fever
  3. agitation
  4. autonomic instability
  5. cognitive changes eg. delirium
  6. associated with elevated creatine phosphokinase (CPK)

A mneumonic used to remember the features of NMS is FEVER.

  • F – Fever
  • E – Encephalopathy
  • V – Vitals unstable
  • E – Elevated enzymes – CPK
  • R – Rigidity of muscles

Treatment:

  • symptom management
  • removal from neuroleptic agent
